Description

During routine operations, the event occurred and was reported immediately to the supervisor on duty. Area was secured and initial response carried out per emergency procedure.

Immediate Actions Taken

Area cordoned off. Affected personnel attended. Supervisor and HSE notified. Work stopped pending investigation.

Root Cause Analysis

Inadequate hazard identification during pre-task review.
